Research Ties Common Heartburn Meds to Kidney Disease

Common medications prescribed to treat heartburn, acid reflux and ulcers are linked to increased risks for kidney failure and chronic kidney disease, found a recent University at Buffalo study.

Use of proton pump inhibitors (PPI), a group of drugs that reduce the production of stomach acid, increases the risk of chronic kidney disease by 20 percent and raises the risk of kidney failure by four times. Risks were highest among people at least 65 years old.

The research, published in February in Pharmacotherapy, is one of the first large, long-term studies to examine the effects of PPIs on kidney function. Researchers examined the health data of more than 190,000 patients over a 15-year period.

“This study adds to a growing list of concerning side effects and adverse outcomes associated with PPIs,” says David Jacobs, PharmD, PhD, lead investigator and assistant professor of pharmacy practice in the UB School of Pharmacy and Pharmaceutical Sciences.

“Given the increasing global use of PPIs, the relationship between PPIs and renal disease could pose a substantial disease and financial burden to the health care system and public health.”

PPIs are one of the most commonly prescribed medications in the U.S., with an estimated 113 million prescriptions filled in 2008, costing patients nearly $14 billion, says Jacobs.

Due to acid reflux and related conditions only requiring short-term treatment with PPIs, he adds, up to 70 percent of patients overuse these medications without benefit and are subjected to unnecessary adverse effects.

The prevalence of PPI use in the U.S. could have a devastating effect on public health. Because these drugs are still considered safe, education and deprescribing initiatives are needed to raise awareness among health care providers, says Jacobs. Deprescribing may involve reducing dosage or stopping usage.

Data for the investigation was gathered from medical insurance and prescription claims from a Western New York insurer. Researchers examined medical history from 1993-2008 of adult patients with no history of kidney disease.

Kidney health was compared between patients who underwent PPI therapy and those who were unexposed. Examined PPIs included esomeprazole, lansoprazole, omeprazole, pantoprazole and rabeprazole (commonly known by brand names as Vimovo, Prevacid, Prilosec, Protonix and Aciphex, respectively).

Three US troops, 1 contractor killed in Afghanistan IED blast, Pentagon says

Three US troops and 1 contractor were killed by an improvised explosive device in Afghanistan Monday near Bagram Air Base, the Pentagon announced. Three U.S. service members were also wounded in the blast.

Those hurt were evacuated and are receiving care, the Pentagon said. The names of those killed are being held back for 24 hours until the notification of next of kin has been completed, as per U.S. Department of Defense policy.

The violence follows recent signs of support of US outreach to the Taliban.

Chief Executive Abdullah Abdullah says he supports U.S. efforts to pursue a cease-fire with the Taliban, who effectively control half of Afghanistan and refuse to negotiate with his government. Abdullah Abdullah shares power with President Ashraf Ghani in a U.S.-brokered unity government. Abdullah spoke to The Associated Press on Sunday at the World Economic Forum in Jordan.

Abdullah says the Taliban could take part in elections and even compete for the presidency if they renounce violence. He says Afghans want peace, but "they don't want to live the way that the Taliban want them to live."

The Taliban have held talks with a U.S. envoy in recent months while continuing to carry out daily attacks on Afghan forces. The insurgents dismiss the Afghan government as a U.S. puppet.

Trial to start over dozens of dead horses at Delaware farm

The owner of a Maryland farm is going to trial after dozens of horses were found dead and decaying as more than 100 others were neglected and starving on her property.

The Daily Times of Salisbury reports 75-year-old Barbara Pilchard is set to appear in court Tuesday morning on charges of animal cruelty, neglect and abuse.

Pilchard was indicted last summer after authorities responding to a report of decaying horse corpses at the 2-acre farm on the state's Eastern Shore saw the severity of the situation.

Wicomico County Sheriff Mike Lewis said previous allegations of horse neglect against Pilchard weren't sufficiently supported by evidence.

He says the living horses were starving and had broken into the home in an attempt to find food. They were placed with rescue groups.

Trump says no plan to revive family separations, blames Obama for uproar

President Trump said Tuesday that the administration has no plans to revive the controversial policy that allowed for family separations at the border, amid renewed speculation about whether the practice could return amid the shakeup at the Homeland Security department.

“We’re not looking to do that,” Trump told reporters on Tuesday prior to a closed-door meeting with Egyptian President Abdel Fattah el-Sisi.

He added that “once you don’t have it, many more people are coming, like a picnic. Like Disney Land.”

But he said, “We’re not looking to do it.”

The president ended the practice under pressure and mounting criticism last year.

But the issue of family separations has emerged once again after DHS Secretary Kirstjen Nielsen's resignation and other changes in the department. The president also withdrew the nomination of acting Immigration and Customs Enforcement (ICE) Director Ron Vitellio to become the permanent head of the agency, telling reporters that “Ron’s a good man, but we’re going in a tougher direction, we want to go in a tougher direction.”

Speaking with reporters on Tuesday, though, Trump sought to pin the blame on his predecessor for the uproar.

“Obama separated the children, just so you understand. President Obama separated the children,” Trump said. “The cages that were shown, very inappropriate, they were built by President Obama and the Obama administration –not by Trump.”

“The press knows it, you know it, we all know it,” he said. “I’m the one that stopped it.”

Initial images of cages with children inside that spread on social media last year indeed were from the Obama administration. The photos, taken in 2014 by The Associated Press, were wrongly described as illustrating imprisonment under the Trump administration.

But while some family separations may have happened under Obama, it was not nearly as widespread as it was during the Trump administration, amid a “zero tolerance” policy that led to the separations.

Neither the Obama administration nor the Trump administration had an explicit family separation policy, but the “zero tolerance” policy meant that anyone caught crossing the border illegally was to be criminally prosecuted, even if they had few or no previous convictions. Under that policy, adults were taken to court for criminal proceedings, while their children were separated from them. If the charges took longer than 72 hours to process, children would be sent from the care of Customs and Border Protection to the Health and Human Services Department.

Last June, Trump signed an executive order that stopped family separations.

Trump also slammed congressional Democrats Tuesday for not doing enough to "act" on border security.

"Homeland security is what we want--there's no better term, there's no better name," Trump said. "We want homeland security and that’s what we’re gonna get."

HANOI (Reuters) – Fast-growing Vietnamese budget airline VietJet Aviation JSC is expected to sign a major jet deal with Boeing Co on the sidelines of next week’s Trump-Kim summit, according to sources familiar with the matter.

U.S. President Donald Trump and North Korean leader Kim Jong Un will hold their second summit in the Vietnamese capital of Hanoi on Feb 27-28.

Holding a signing ceremony during Trump’s visit would help emphasize strengthening economic and military ties between the United States and Vietnam.

VietJet, while not government-owned, increasingly uses state visits to showcase major plane orders balanced between Boeing and Airbus SE. It signed a deal to buy 100 Boeing 737 MAX narrowbody jets when former U.S. President Barack Obama visited Hanoi in 2016.

The airline is likely to finalize next week a separate provisional deal agreed last year at the Farnborough Airshow to buy another 100 Boeing 737 MAX jets worth almost $13 billion at list prices, sources said on condition of anonymity due to an expected announcement by VietJet.

The U.S. Federal Aviation Administration declared last week that Vietnam complied with international aviation standards, in a move that would allow Vietnamese carriers to fly there for the first time and codeshare with U.S. airlines.

VietJet said last week it planned to purchase widebody jets capable of U.S. flights to open routes to cities with large Vietnamese communities in the United States, such as in California.

It might be too early for VietJet to place a widebody order, said one of the sources.

Another source briefed on the matter said the deal for 100 737 MAX jets was already on Boeing’s books, having been firmed up earlier and listed as an unidentified customer.

Boeing declined to comment. VietJet did not respond immediately to a request for comment.

VietJet finalised a deal in November with Airbus for 50 A321neo jets during a visit to Hanoi by French Prime Minister Edouard Philippe that had also been announced provisionally at the Farnborough Airshow.

VietJet operates 385 flights daily within Vietnam and to places such as Japan, Hong Kong, South Korea, Taiwan, Singapore, China, Thailand, Myanmar and Malaysia.

However, industry analysts have questioned whether the airline will take delivery of all the aircraft on order as the aerospace industry reaches the peak of an extended growth phase.

Biden accused by second woman of improper physical contact

A second woman has come forward to accuse former Vice President Joe Biden of inappropriate touching, further complicating the prospective presidential candidate's political future and intensifying what has become a social media reckoning on his previous public behavior with women.

Amy Lappos, 43, told the Hartford Courant in an interview published Monday that Biden grabbed her during a $1,000-per-plate October 2009 fundraiser for Connecticut Democrat Rep. Jim Himes. Lappos, who initially posted about the episode on Facebook on Sunday, was working as an aide for Himes at the time.

“It wasn’t sexual, but he did grab me by the head," Lappos told the paper. “He put his hand around my neck and pulled me in to rub noses with me. When he was pulling me in, I thought he was going to kiss me on the mouth.”

A Biden spokesman did not immediately respond to Fox News' requests for comment. In statements earlier Monday and over the weekend, both Biden and his team strongly denied accusations the former vice president acted inappropriately around women. Still, they vowed to listen to any allegations or concerns.

Lappos, who was seen in photos with Biden at the event, said the episode occurred inside the Greenwich home of an affordable housing developer, and that Himes was not present at the time.

Lappos continued: “I never filed a complaint, to be honest, because he was the vice president. I was a nobody. There’s absolutely a line of decency. There’s a line of respect. Crossing that line is not grandfatherly. It’s not cultural. It’s not affection. It’s sexism or misogyny.”

In her initial Facebook post, Lappos condemned those who would suggest the "grossness of men invading women’s boundaries to sniff and touch is debatable. And shame on some of the women here excusing this as boys being boys or it’s not as bad as Trump. I can speak from experience when I say it’s an incredibly uncomfortable situation and not at all acceptable. We need to hold our men to the same standards we hold all men."

Lappos' accusations came on the heels of a similar claim by former Nevada lieutenant governor candidate Lucy Flores, who accused Biden of inappropriate sexual conduct during a 2014 campaign event. Flores alleged in an article Friday that Biden "plant[ed] a big slow kiss on the back of my head."

In a statement shortly afterward, a Biden spokesperson denied that the vice president or his staff had been aware that Flores was uncomfortable. And the 76-year-old prospective 2020 presidential candidate issued a statement Sunday strongly denying wrongdoing -- but promising to "listen respectfully" to any accusations of misconduct.

Flores' claim prompted a slew of social media posts highlighting what critics said amounted to Biden's clearly inappropriate behavior in public view.  Some images showed Biden alongside women ranging from a biker to the wife of the former defense secretary to actress Eva Longoria – raising questions about whether the images will haunt Biden in the #MeToo era as he moves toward a rumored 2020 presidential run.

"I'm just shocked that no one has ever told Vice President Biden, 'stop interacting with women that way. You're making them feel uncomfortable,'" Flores told MSNBC on Monday. "Or, at least we think you’re making them feel uncomfortable because that is the perception we’re seeing from the outside in."

For her part, Lappos told the Courant that Biden's apparent behavior was disqualifying: "If Biden truly supports women and gender equality he would step aside and support one of the many talented and qualified women running. The same goes for the other men who have thrown their hat in the ring. Women are 52 percent of the population. We are not a minority, we are the majority. It is time we are represented as such. After 45 male presidents it is time we elect a woman."

But, in a fiery statement Monday, a Biden spokesman pushed back on the accusations made against him at that point, saying many photos of Biden have been misinterpreted -- even according to some of the women who appear in them -- while others were blatant "smears and forgeries."

"One such instance involved Stephanie Carter, the wife of former Defense Secretary Ash Carter, and the other Senator Chris Coons’ daughter," Biden spokesman Bill Russo said. "Each was ostensibly captured by a photo that some have interpreted in a way that is reflected consistently, and without scrutiny, in the reporting. Ms. Flores herself has cited the Carter photo as an example of the behavior she experienced.

"Here’s the problem: in neither case is the often repeated and recirculated interpretation true," Russo continued. "Both Stephanie Carter and Senator Coons have now felt compelled to speak out to put these ugly urban legends to rest."

Russo cited an interview in which Carter corrected the record on her own experience with Biden, as well as a Washington Post article in which Coons' daughter similarly absolved Biden of misconduct.

"And they are not the only ones," Russo said. "There are other, even more insidious examples of claims about the Vice President that have no foundation: the use of photoshopped images and other manipulations of social media. Perhaps most galling of all, a cropped photo of the Vice President comforting his grandson outside of his son Beau’s funeral has been used to further this false narrative."

Russo concluded: "The important conversation about these issues are not advanced, nor are any criticisms of Vice President Biden validated, by the continued misrepresentation of the Carter and Coons moments, or a failure to be vigilant about a cottage industry of lies."

NEW DELHI (Reuters) – Surging global oil prices will pose a first big challenge to India’s new government, whoever wins an election now under way, especially as domestic prices have been allowed to lag, meaning consumers are in for a painful surge as they catch up.

For oil-import dependent India, higher global prices could lead to a weaker rupee, higher inflation, the ruling out of interest rate cuts and could further weigh on twin current account and budget deficits, economists warned.

But compounding the future pain, state-run fuel suppliers and retailers have held off passing on to consumers the higher prices during a staggered general election, which began on April 11 and ends on May 23, according to sources familiar with the situation.

That delay is expected to be unwound once the election is over. And there could be additional price increases to make up for losses or profits missed during the period of delayed increases, the sources said.

In some major Asian countries, such as Japan and South Korea, pump prices are adjusted periodically so they move largely in tandem with international crude prices.

That was what was supposed to happen in India but the election means there have been many days when pump prices have been unchanged.

In New Delhi, for example, while crude oil prices have gone up by nearly $9 a barrel, or about 12 percent, in the past six weeks, gasoline prices have only risen by 0.47 rupees a liter, or 0.6 percent.

State-controlled fuel suppliers and retailers declined to say why they had delayed price increases, or discuss whether there has been any pressure from the government of Prime Minister Narendra Modi.

A government spokesman declined to comment.

The opposition Congress party said Modi’s government was violating its own policy of daily price revision by advising the state oil companies to hold prices steady.

“The government should cut fuel taxes otherwise consumers will have to pay much higher oil prices once the elections are over,” said Akhilesh Pratap Singh, a senior leader of the Congress party.

Nitin Goyal, treasurer at the All India Petroleum Dealers Association, representing fuel stations in 25 states, said prices were similarly held down for 19 days in the southern state of Karnataka last year, when it held state assembly elections.

Only for them to surge after the vote.

“Consumers should be ready for a rude shock of a massive jump in retail prices, similar to the level we have seen in the Karnataka state election,” Goyal said.

‘CREDIT NEGATIVE’

Sri Paravaikkarasu, director for Asia oil at Singapore-based consultancy FGE, said retail prices of gasoline and gasoil prices would have been up to 6 percent, or about 4 rupee, higher if they had been allowed to rise in line with global prices.

“Indian pump prices have failed to keep up with the recent uptrend in crude prices,” Paravaikkarasu said.

“With the country’s general elections underway, the incumbent government has been keeping pump prices relatively unchanged.”

India had switched to a daily price revision in June 2017 from a revision every two weeks, as the government allowed retailers to set prices.

But the government faced protests last October when retailers raised prices by up to 10 rupees a liter after the crude oil price went above $80 a barrel, forcing it to cut fuel taxes.

Global prices rose to their highest level in 2019 on Thursday, days after the United States announced all Iran sanction waivers would end by May, pressuring importers including India to stop buying Tehran’s oil. [O/R]

Higher oil prices will mean Asia’s third largest economy is likely to see growth of less than 7 percent rate this fiscal year, economists said. Growth slowed to 6.6 percent in the October-December quarter, the slowest in five quarters.

Rating agency CARE has warned that a 10 percent rise in global oil prices could increase demand for dollars, putting pressure on the rupee and widening the current account deficit.

India’s oil import bill rose by nearly one-third in the fiscal year ending March 31 to $140.5 billion, against $108 billion the previous year.

“The increase in international oil prices is a credit negative for the Indian economy,” ICRA, the Indian arm of the Fitch rating agency, said in a note.

“Every $10/ bbl increase in crude oil prices increases the fiscal deficit by about 0.1 percent of GDP.”

Any big price rise would also build a case for the central bank to keep rates steady, or even raise them.

The Reserve Bank of India’s Monetary Policy Committee, which cut the benchmark policy repo rate by 25 basis points this month, warned that rising oil and food prices could push up inflation.

Policymakers are worried that a sustained increase in the oil price in the range of $70-75/barrel or higher can move the rupee down by 3-4 percent on an annual basis.

The rupee has depreciated by 1.24 percent against the dollar since a year high in mid-March.

(Reuters) – Ride-hailing company Uber Technologies Inc unveiled terms for its initial public offering on Friday, telling investors it would seek to sell as much as $10.35 billion in stock at a valuation of up to $91.5 billion.

In a regulatory filing, Uber set a target price range of $44-$50 per share for its IPO. The company will sell 180 million shares in the offering, with a further 27 million sold by insiders.

In the filing, Uber also reported a net loss attributable to the company for the first quarter of 2019 of around $1 billion and revenues of roughly $3 billion.

NEW DELHI/MUMBAI (Reuters) – The grounding of India’s Jet Airways is turning into a quick windfall and long-term opportunity for international airlines keen to scoop up nearly a million outbound passengers from what was once the nation’s biggest airline.

Jet, which previously had a fleet of around 120 largely Boeing Co planes, was forced to indefinitely halt all flight operations on April 17 after its banks rejected the carrier’s plea for emergency funds.

The carrier’s descent into crisis has benefited international airlines in the form of rising fares and demand, data showed.

Fares from India to cities such as Dubai, London, New York, Singapore and Bali in the first quarter of 2019 rose between 4 percent and 32 percent from a year ago, according to Indian travel portal MakeMyTrip Ltd.

In the peak travel months of May and June, fares to London have spiked as much as 36 percent and tickets to San Francisco are up nearly 20 percent from a year ago, according to data from travel portal Yatra.com.

“For the next three months it’s actually bonanza time for international players,” said Ashish Nainan, a research analyst at CARE Ratings. “At least until the middle of June, the fares are not going to come down.”

Due to rising demand, even before Jet’s lessors grounded planes, carriers such as British Airways, Cathay Pacific Airways Ltd, Singapore Airlines Ltd and United Airlines saw an up to a 27 percent increase in passenger numbers from India in the last quarter of 2018, data from India’s aviation regulator showed. That is the latest period for which the data is available.

India is one of the world’s fastest-growing aviation markets, clocking 15-20 percent domestic growth in recent years. It has long had only two full-service long-haul carriers, state-run Air India and Jet.

Jet is now hoping to be bailed out by a new investor, with final bids due on May 10.

INCREASING CAPACITY

Before its grounding, Jet had the biggest share of India’s outbound international air traffic, carrying 12 percent of the 7.8 million passengers headed overseas in the Oct-Dec quarter, down from 14 percent a year earlier, data from the Directorate General of Civil Aviation showed.

The total number of passengers traveling overseas with Jet fell 10 percent during the last quarter of 2018 even as the outbound travel market grew about 5 percent.

Meanwhile, Singapore Airlines posted a 27 percent increase in passengers from India, Cathay registered 17 percent growth and British Airways saw a 10 percent rise in the same period.

Cathay said the events at Jet combined with increasing demand for travel had led it to deploy larger aircraft with more seats on some Indian routes.

“In the long term we would certainly like to be able to offer more capacity into India, not just on our existing routes but by establishing new services to secondary cities,” Cathay said in a statement.

Singapore Airlines, in an email to Reuters, said the Indian market is “very promising” but declined to give details of airfare levels or demand patterns in the wake of Jet’s exit, citing a quiet period before the release of its annual results.

DOMESTIC GAINS

Jet’s grounding has also had a big impact on the domestic market, with inter-city air fares to major cities such as New Delhi, Mumbai, Bengaluru and Kolkata soaring more than 20 percent in May and June, according to Yatra.com.

The spike in fares is expected to underpin strong earnings for IndiGo and SpiceJet Ltd, which are set to report results for the quarter ended March 31 in the coming weeks.

“Domestic Indian carriers are the main benefactors, but I suspect if Jet fails to be revived by May 10 then Vistara and other airlines that ply international routes, particularly the lucrative Gulf market, are the main winners,” said Shukor Yusof, the head of aviation consultancy Endau Analytics. Vistara is a joint venture of India’s Tata Sons and Singapore Airlines.

Inadequate bilateral traffic rights between India and other countries, however, could be an impediment to foreign carriers’ hopes of winning business lost by Jet, some analysts said.

“Even before Jet’s operational shutdown, international capacity was significantly constrained,” said Kapil Kaul, CEO for South Asia of consultancy CAPA. “We have now more serious capacity challenge … this is unlikely to be stabilized in the near term.”

A new national government likely to be in place sometime after elections end in May is expected to address the international capacity constraints, and once bilateral agreements are eased airlines including Emirates, Turkish and Qatar would immediately benefit, said Kaul.

“We would love to add more flights but we are at the limit of the allocation granted to us for traffic rights,” Emirates Chief Commercial Officer Thierry Antinori told reporters in Dubai on Wednesday.

(Reuters) – AstraZeneca Plc beat first-quarter sales and earnings expectations on Friday as the British drugmaker benefited from a push into cancer drugs and emerging markets including China.

Newer treatments such as lung cancer drug Tagrisso, now the company’s top selling medicine, have helped the drugmaker’s return to growth after years of crumbling sales due to patent losses on older drugs.

Sales in China have shown explosive growth, more than doubling since 2012, but AstraZeneca executives on Friday said that may not be sustained.

“The enormous growth you currently see in China, 28 percent, probably is not sustainable, but we feel very bullish that the growth will continue to be at a pace of between 15 percent and 20 percent,” Ruud Dobber, executive vice president, BioPharma, told Reuters.

Shares of the company were down 0.2 percent at 5,878 pence at 1031 GMT.

The turnaround in AstraZeneca’s fortunes has been powered by a push into cancer treatments led by Chief Executive Pascal Soriot, who saw off a 2014 takeover bid from Pfizer in part by promising annual sales of $45 billion by 2023.

In the first quarter, sales from its oncology unit rose 59 percent to $1.89 billion, accounting for 35 percent of total product sales.

The company has moved deeper into cancer therapy market through wide-ranging deals, including those for immunotherapy and targeted therapy. Last month, it agreed a multi-billion dollar oncology deal with Japan’s Daiichi Sankyo Co Ltd.

“We’re reaching that point where after years of having to keep faith, we have actually got something tangible to believe in,” Hargreaves Lansdown analyst Nicholas Hyett said.

AstraZeneca also backed its annual sales and earnings forecast and said it has extensively prepared for UK’s anticipated exit from the European Union, even in the event of a no-deal exit.

The company has already spent more than 40 million pounds ($52 million) on Brexit preparations, including stockpiling six weeks’ worth of drugs in the UK and four weeks in continental Europe to guard against shortages.

AstraZeneca said product sales rose 14 percent at constant currency to $5.47 billion in the quarter, led by its lung cancer drug Tagrisso and respiratory treatment Pulmicort.

China sales increased by 28 percent to $1.24 billion in the quarter, accounting for nearly a quarter of overall product sales.

Core earnings came in at 89 cents per share in the quarter. Analysts on average were expecting core earnings of 85 cents per share and product sales of $5.29 billion, according to a company provided consensus of 19 analysts.
